Overview: Compile a survey rough draft on Google Forms
The purpose of this is to send me a rough draft of your survey. Once I approve it, you can send it out. The sooner you send it out, the sooner you can begin collecting data.
Please include with your submission (not in the Google Form itself, but in the submission textbox along with the survey URL)…
- The final RQs which we discussed in research teams last week
- List each variable and for each variable and include the questions you will use to measure that variable.
- For example, if my variable is depression, I might use Beck’s Depression Inventory, the Center for Epidemiologic Studies Depression Scale, or the Hamilton Depression Rating Scale. Please include a link to the scale and the article in which you saw it used.
- Conclude with demographic questions that might be helpful in answering your RQ (e.g., age, gender, race/ethnicity, political affiliation, highest attained degree)
Compile on a Google Form (preferable) and submit link. Alternatively, you can upload to a Microsoft Word, Pages, or Google Document, then export as a PDF once finished.

Assignment Example
We read Xaio (2019) earlier this semester. Her constructs (you can see in her measures section on page 5) were affective attitude, cognitive attitude, perceived susceptibility to HPV, perceived severity of HPV, and Intention to get the HPV vaccine. Each one of those variables had a different measurement.
Let’s say she was doing this assignment. An example of variable label would be:
Variable: Perceived susceptibility to HPV
Scale: 3 item, 7-point from “Strongly disagree” to “strongly agree”
Items:
1. “It is likely I will contract HPV”
2. “I am at risk for getting genital HPV”
3. “It is possible that I will get HPV”
